[PHM] Integrate authentication with a single provider for any user

Currently, PHM is integrated with the Rotherham Azure AD. If other users that are not in that directory require access, development work has to take place to use another source of integration. We should look at using a single source such as NHS Identity or if we can federate other AD instances through Google.
